Former title: The Comprehensive Assessment and Case Planning. This is a 2-day webinar. The case plan is a partnership agreement between the case manager and the client. Its development and implementation require a complex set of activities usually involving several areas of need and several providers. Participants in this interactive webinar will use a case study assessment from which reasonable, specific and measurable goals will be developed as a first step in formulating a case plan. Resource identification, linkages to services, advocacy to eliminate barriers, as well as the complexities of the case management relationship and ethical dilemmas will be explored as the plan moves into the intervention phase. Follow-up, monitoring, interagency coordination, and reassessment will allow the participant to determine whether the plan developed yesterday, still works for the client today. And finally, outcome evaluation will let the participants assess whether the client has measurably benefited from the case management relationship. Attendees must attend both days to receive a certificate of completion. [Required for Case Management Certificate Program]<p>

To earn CE credit, social workers must log in at the scheduled time, attend the entire course and complete an online course evaluation. Certificates of completion will be emailed within 10 business days of course completion.
